Congratulations on your decision to apply to the Central Susquehanna LPN Career Center!

Follow the application steps below, and contact the LPN Career Center if you have any questions. 

1. Complete the application (also available in the program catalog) and mail it with the application fee, along with two letters of reference, to:

2. Submit your official high school or official GED transcript.
   
   
3. Upon receipt of the completed application, students will be directed to take the Test of Essential Academic Skills (TEAS) entrance exam. Information about the TEAS will be sent after a completed application is obtained.
   
   
4. The Admissions, Academics, and Ethics Committee reviews all applications for a recommendation of admission. The deadline for submitting an application for the full-time program is May 31st and the deadline for submitting an application for the part-time program is October 15th. Please call for specific deadlines and admission decision dates.
   
   
5. Applicants accepted into the nursing program who have attended other academic institutions may submit an official transcript from that institution to apply for credit for courses taken.
